Our health system implemented an innovative high-intensity home-based rehabilitation model—“Home Care Plus” (HCP)—in 2019 for patients attributed to our ACO. The primary aim of this presentation is to examine functional status at PAC discharge for patients in HCP versus a SNF. We also compare hospital length of stay, 30-day readmission, and 30- and 90-day post-hospital healthcare costs between HCP and SNF cohorts. In this session we will discuss our vision, goals, inclusion and exclusion criteria, operational challenges and successes, as well as outcomes. In the end, we believe we are driving VALUE.
