With healthcare professionals continuing to undervalue the stories that lie behind every patient, we only continue to fail in providing better patient outcomes. Hospital readmission rates and emergency room visits remain high, requiring CMS to take further action in incentivizing agencies thru Value Based Purchasing. Agencies, no doubt, heal wounds and maximize potential, but what happens after discharge? Why are we finding these patients back at our door? Patients are people, like you and me. We belong to a certain demographic that influence how we live. We encounter real-life challenges that already make life difficult as it is. But some of these pose as obstacles to our health. A person with CHF who is short of breath may not have the ability to prepare a low sodium meal. Their priority of living is greater than a low sodium diet, which could mean that TV dinners becoming staple in that household. Life happens at home. Treating the illness is only a tiny part of what we do.
