Description:
Hear Dr. Jennifer Kennedy, Vice President of Quality and Standards for CHAP review the components for certification and the value to hospice and home health agencies designated as Age-Friendly Care at Home providers. Most people want to receive care at home as they age. Community Health Accreditation Partners recently launched Age-Friendly Care at Home certification as an extension of the Age-Friendly Health Systems program, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and Catholic Health Association of the United States. To be certified, a provider must support a care model that follows four evidence-based elements of high-quality care, known as the “4Ms”—what matters most, medication, mentation, and mobility.

 

Learning Objective 1:
Identify the core components of the Age-Friendly Care at Home model, including the 4Ms, and how to apply them across the care team.

Learning Objective 2:
Obtain necessary background information about the required quality standards and process for pursuing Age-Friendly Care at Home certification so they will be equipped to begin the process at their program or agency.

Learning Objective 3:
Explain how to apply Age-Friendly Care at Home certification requirements to improve existing care delivery processes. They will hear from the Vice President of Clinical Quality and Standards from the first hospice in the country to achieve Age-Friendly Care at Home certification, about their experience and the value of certification.
