Many agencies have dedicated resources to develop strategies to anticipate and prevent errors from reaching patients. Studies reveal that insufficient communication among team members and during transitions of care significantly contribute to adverse events. A HRO anticipates and prevents medical errors to achieve reliable patient care outcomes. The value of healthcare increases by improving quality, decreasing cost, driving out variation and designing process improvement measures This session will guide Home Care/Hospice organizations towards safer patient-centered transitions of care by using the basic principles and strategies of a HRO.

- Describe the three (3) basic principles of a High Reliability Organization: Transparency, Just Culture, and Patient Engagement.
- Explain how error prevention promotes Population Health strategies: safe transitions of care, reduction of re-admissions, LOS, complications, and health care costs.
- Describe effective strategies to involve staff at all levels to sustain the tenets of a HRO: Speak up Culture, Patient Advocacy Shared Stories, Good Catches, Safety Coaches, and Leadership Rounding.
