Many care delivery systems are now developing internal programs for the delivery of end of life care, excluding traditional hospice agencies. Existing hospice agencies will need to reinvent their organizations and restructure their relationships to maintain their important position in the health care continuum. This session will help hospices catalog skills and identify opportunities to leverage their resources and promote strategies for program development and engagement with health care systems, ACO’s, and IDN’s. The presentation will include examples of innovative hospice delivery models, and will also discuss options of palliative service integration into hospice organizations.

- Identify health care system changes that result in the development of end of life programing that excludes traditional hospice agencies in end of life care.
- Explain alternative and innovative models of hospice delivery.
- Define options for hospices to integrate and affiliate with palliative programs.
