As the population of older Americans with chronic conditions continues to grow, the role of home health care (HHC) services in improving care transitions between acute care and independent living has become a national priority. This has resulted in the development of a number of value-based purchasing (VBP) initiatives, changes in the Centers for Medicare and Medicaid Services’ Home Health Conditions of Participation, and The Joint Commission’s focus on infection prevention as a national patient safety goal for HHC. In this presentation, we will report the results from a national study of home health care providers describing the challenges to infection prevention and quality improvement in HHC, as well as lessons learned.

 Learning Outcomes:

  • Describe the current infection prevention and control infrastructure and policies in HHC agencies in the U.S.
  • Identify the incidence of infections that occur while patients are receiving HHC.
  • Explain how home health care agencies have responded to various quality and VBP initiatives.
