604. How Home-Based Programs Partner to Address Universe of Post-Acute Demands

604. How Home-Based Programs Partner to Address Universe of Post-Acute Demands is proudly presented to you by National Association for Home Care & Hospice. Thank you. We hope that you enjoy your course.

Description

All organizations are seeking creative ways to provide the right level of care at the right time to ensure quality outcomes and patient satisfaction. Additionally, it is increasingly important to coordinate programs longitudinally to better manage total cost of care and reduce avoidable hospital readmissions. This is true in risk-based programs with payors or to meet the demands of health systems. The ability to provide multiple levels of care in the home is central to satisfying all stakeholders. This session explores how several organizations are addressing the opportunity through joint ventures or other structures – including personal care, home health, hospice, palliative care, SNF at home, hospital at home, or some combination of these programs.  Education

Learning Objectives

• Identify programs developed, including traditional joint ventures
coordinating home health and hospice, along with other innovative
programs addressing longitudinal care and higher acuity programs in the
home.
• Examine program design and technology utilized to achieve outcomes
desired by patients, health systems, and payors.
• Share examples of structural options used to build these programs and
partnerships.
