Description:

The purpose of the HHVBP Workgroups at the NYU Langone Certified Home Health Agency are to utilize an interdisciplinary approach to develop and implement evidence-based strategies to decrease acute care hospitalizations by identifying opportunities to improve, organizational practices, data analysis, and educational approaches. The 60-day hospitalizations decreased 41% as a result of the workgroup. The NYU CHHA is now 5 stars for hospitalizations and lowest in New York State.

Learning Objective 1:
Discuss the HHVBP Model for certified home health agencies and how quality is measured.

Learning Objective 2:
Explain the value of the interdisciplinary workgroup model as a successful quality improvement strategy to address hospitalization metrics including re-hospitalizations and emergency department (ED) visits.

Identify successful interventions to prevent re-hospitalizations and ED visits.
