Description:
The need for home-based palliative care (PC) is rising as more home health care (HHC) patients have multiple chronic conditions. However, only 7% of PC programs are run by HHC agencies due to reimbursement restrictions. If reimbursed, PC services could improve HHC patient outcomes. Understanding clinician readiness and patient/caregiver receptiveness to PC is needed. Pilot study findings assessing these factors among HHC clinicians, patients and caregivers (via questionnaires) will be presented.

Summarize the eight domains of palliative care from the NCP Guidelines for Quality Palliative Care and related evidence-based practices for providing quality palliative care for seriously ill adults in the HHC setting.
