Thorough hospice RN assessments that capture the “why” each patient is electing hospice AND clearly support hospice eligibility are essential to support eligibility throughout the hospice election and guide the IDG and Plan of Care updates to ensure the patient and their caregivers needs are met. This session presents tips and guidance that clearly support the patient's terminal prognosis, address all required elements and meet the conditions of participation.
