Home Health and Hospice Value Proposition Using Data is proudly presented to you by National Association for Home Care & Hospice. Thank you. We hope that you enjoy your course.
Home Health and Hospice agencies have transformed in the last few years in how they participate in the continuum of care. Providers see a greater focus on outcomes and financial performance related to their referral relationships and patterns. As payment reform continues to shape the health care landscape providers are becoming heavily dependent upon knowing how their quality and episode spending data compares to their peers in the market. This session will focus on key quality and financial data elements that drive communication and collaboration across the continuum of care.
Learning Outcomes:
- Define the environmental factors that are changing behavior in the continuum of care.
- Identify key components of an influential value proposition.
