Documentation audits can seem like a chore. Whether it's a technical audit for billing or content audit for appeal avoidance, the tendency of an auditor to become lulled into a false sense of security is sometimes overwhelming. This presentation will review best practices for documentation audit processes. We will examine the CMS requirements for documentation quality including skilled services, medical necessity and the objective measurements of progress as it pertains to therapy. We will engage the participants in case reviews to see if they can spot documentation shortfalls. We will also review the Review Choice Demonstration and how that may impact your agency in the months or years to come.
