COPD affects 15.7 million adults. COPD patients are high utilizers of healthcare and frequently hospitalized. Under PDGM home health will need to reevaluate models of care for new efficiency and outcome gains. This piloted care model can also be extrapolated across disease types to manage other patient populations This session will describe how program participants are identified, enrolled and managed with a coordinated team approach incorporating telehealth to empowers patients’ self-management. Clinical outcomes of an analysis evaluating the impact of the program on re-admissions will be presented.
