See how home health agencies can use “step-down” telehealth to improve long-term patient health and increase their reimbursement with the expansion of pay for performance. Step-down solutions follow the traditional patient monitoring period and allow patients to track their own vital signs with consumer-focused telehealth technology. This is a task they are ideally for, according to the experience of VNA Rockford, which found that patients had picked up knowledge about vital signs from home health staff. The influx of consumer wearables has enabled patients to put this knowledge to good use in monitoring their health and encouraged Rockford to launch a pilot program using step-down solutions.
