Increasingly, strategic referral partners for home health and hospice are showing a strong interest in palliative care services. With the prospect of narrowing networks, palliative care may become an important distinguishing mark if not a requirement for agencies that wish to partner with health systems, physician networks, and/or commercial payors. This session will focus on examples of palliative care programs that home care providers are launching to meet the needs of these key strategic partners. Learn what agencies and their referral partners have to say about the challenges and rewards of setting up these programs, with a focus on the issue of how much to customize programs to suit individual partners’ specific needs.
