Care Planning in LTC Nursing Facilities

A resident care plan is a communication tool that guides all members of the healthcare team on how to meet each resident’s needs. It was first introduced in the 1900s to educate nursing students on how to provide holistic and individualized care to hospital patients. Care plans have since evolved into a standard of practice for nurses, social services, and other interdisciplinary team members in different types of care settings. This course discusses the purpose of the care plan. It also discusses the steps in the care planning process and how to make the care plan resident-centered.
