Patient and caregiver empowerment methodologies, applied at home, can help high-risk heart failure patients remain in their homes and prevent hospitalizations. Patients using an interdisciplinary approach teaching self-management strategies are able to live a better life, have better health outcomes, and organizations find their hospitalization rates improve by up to 8 percent.
