Home care and hospice organizations have great potential for fostering more affordable, high-quality care in accountable care organizations (ACOs). But what will it take for post-acute providers to effectively capture and demonstrate their value in the global quest to better manage the health of communities and designated populations? In this session, we will explore ways to develop collaborative relationships that build and sustain ACOs, while improving internal processes and procedures for excelling in a value-based healthcare environment.

Learning Objectives

• Describe the main focus of ACOs in order to make a plan for building relationships that will improve an organization’s quality, finances, and effectiveness
• Identify the key components of data collection, quality reporting, and financial analysis to track and demonstrate value to ACO partners
• Develop tools and processes to compare performance against best practice benchmarks
• Review processes and procedures to ensure streamlined, high-quality operations
• Make a plan to train employees so they develop a higher level of competency, accountability, and collaboration with ACOs
