The presenters will describe how a home care and primary care clinic partnership improves outcomes. They will discuss how the use of a shared IDT has decreased re-hospitalization for both home care and the physicians, improved OASIS outcomes, improved home care case mix, decreased LUPA rate and increased hospice referrals. They will discuss how this structure is also responsible for implementation of ACO waiver opportunities.
