It aims to improve on a proposed rule from 2015 by providing better care for patients who undergo hip and knee replacements while saving Medicare money. The new CJR model will use bundled payment and hold hospitals accountable for the cost and quality of care they provide. To meet the model’s demands, hospitals are looking for CJR partners. So home health agencies that provide post-operative care to joint replacement patients need to know what arrangements this entails. Failure to become a CJR partner may lead to fewer referrals.
