This session focuses on the need for Patient- and family-centered focus on diagnosis. The presenters will describe how the goals must include; separate goals for each identified problem which are mutually agreed upon by patient, family, team, and healthcare providers. These goals must be measurable, describing the quality, quantity, severity, and frequency of interventions. These goals must be time-limited: short term, long term and realistic cost-effective.

- Identify and describe the services and scope of care offered through the psychiatrist home health nursing
- Understand the impact of mental illness on overall health, increased cost for other health condition and risk of self-harm
- Learn evidence-based approaches to engage patients and families in participating and following the care plan
