This session will help organizations define the term "defensible documentation."
We will help the attendee understand the purpose of providing staff the knowledge to complete defensible documentation. We will help you to implement measures to safeguard clinical documentation, teaching them to become competent in utilizing the tool provided. You will learn how to develop follow-up processes and review of episodes to ensure clinician understanding.

•Explain the purpose in providing defensible documentation in a succinct and accurate patient clinical record
•Educate staff on the importance and rationale behind defensible documentation
• Implement measures to safeguard clinical documentation when under review scrutiny
• Identify the documentation requirements for Medicare and other payers and the consequences in the form of RAC, ZPIC, UPIC and other audits for failing to have defensible documentation
