Continuity of Care and Palliative Care in Serious Illness

Advocating for palliative care early-access and facilitating reflective discussion with patients and families is essential to settle the chaos inherent to palliative care. In the absence of a clear understanding of the degree of illness and level of functional decline, patients and their families do not know what to expect or how to respond to symptom burdens or decision crises. These patients and families face uncertainty and anxiety, and often present multiple times to varied care settings with symptom exacerbations, functional losses, and multiple stressors. Regardless of the care setting, both palliative care teams and interprofessional teams are continually challenged to meet with patients and families to settle chaos, realign goals, and match available resources for care. Sustaining early palliative care access and continuity for all patients facing serious illness requires diligent collaboration and follow up. The goal of this webinar is to provide some practical examples of palliative communication approaches and relevant practice references that staff might use in their own settings. It presents an exploratory review of trends and patterns of palliative care, and highlights the complexity of care and the dilemma of significant gaps that exist in patient understanding, which often results in insufficient decision support and unclear goals for care.
