Learn how to create a seamless care continuum by putting hospice facilities where they are truly needed. Aging and moving through multiple stages of care can be very stressful. Getting access to treatment can be overwhelming for elderly patients and their caregivers. Thankfully, one organization has raised the bar in meeting their needs, while building a financially viable business model. Community Hospice of Northeast Florida is at the forefront in the design of hospice units that enhance the experience of aging and end-of-life for patients, families, and staff. This session will present the business case for hospital-based hospice units, the challenges of designing for the aged, and how to integrate supportive design features into other settings.
