Hear success stories from a home health agency in rural Maine that used telemonitoring to serve the chronically ill. Learn about the challenges it faced with regard to technology development and connectivity issues in rural areas, especially the many that lack cellular reception. This session will discuss the outcomes of grant-funded pilots to reduce rehospitalization and promote use of technology. It will share experiences in the use of evidence-based practices, like patient teaching tools and physician order sets. There will also be a comparison of rehospitalization rates for patients who chose to participate in the program with those who decided to opt out.
