This session will review the star rating systems and describe the elements of best practice models that manage visit utilization, achieve positive patient outcomes, and get high five-star results.

• Identify an effective episode management model that successfully handles visit utilization and clinical outcome achievement to produce positive patient satisfaction (HH-CAHP) scores
